Why do we need personally identifying information?
Your privacy is very important to us. In order to accurately link your personal medical information with your MyLVHN account, you will be asked to provide personally identifiable information (Social Security number etc.). Without this information, there will be a delay in your ability to see upcoming appointments, communicate with your health care provider or request a medication refill.

Have you signed up for MyLVHN before?
If you have already created a MyLVHN account in the MyLVHN mobile app or other means, please do not create a new account. Click the “Sign In” button and use the same sign in information.
If you cannot remember your sign-in information, please select “Forgot Username or Password” from the main login screen for assistance or call us at 844-4MY-LVHN (844-469-5846).
MyLVHN is Your Path to Vaccination
Everyone with a MyLVHN account will automatically be preregistered for a COVID-19 vaccine. You'll receive a notification when you are eligible for the COVID-19 vaccine and can schedule your vaccine appointment. If you are an essential worker fill out the essential worker questionnaire, which can be found by clicking on the menu button after you log into MyLVHN.
