What types of appointments may be scheduled through MyLVHN?
Appointments may be scheduled through MyLVHN with a provider (physician, certified registered nurse practitioner, physician assistant, other advanced practice clinician,) whom you have seen before as a patient. Appointment types include:

You may only schedule one appointment time per provider.
To enroll in MyLVHN, click the "Create Account" button (on right side of this page.) To complete this online form, you will need your personal activation code. Find it on the after-visit summary you received at a recent visit with your LVPG provider. If you do not have the activation code, you can request one using this form: Request an Activation Code.
Find both numbers on the after-visit summary you received at a recent visit with your LVPG provider.
After you have your personal activation code fill out the online "Create Account" form and submit it.
You can also activate your account during a visit with your LVPG provider.
Note: You cannot use the MyLVHN smartphone app to sign up for MyLVHN.The app can only be used to log-into an already-activated MyLVHN account.

I had a MyLVHN account before - is this the same thing?
No, this is a completely new and more robust patient website. You will need to create a new MyLVHN account with an activation code you can receive online or at a patient visit on or after Feb. 18, 2015.
Can I sign into MyLVHN from my smartphone?
Yes. Use your smartphone's browser to visit MyLVHN.org. At the bottom of the page, tap either the Apple App Store for your iPhone or Google Play button for your Android phone. Download the free app for your system and follow the log-in procedure. Note: You cannot use the app to sign up for MyLVHN. Instead, from the front page of MyLVHN.org, follow the "create an account" instructions on the top right side of the page.
Can I view a family member's health record in MyLVHN?
If you're the primary caregiver for children or an adult family member, and already have a MyLVHN account, you may receive proxy (approved guest) access to view portions of their medical information. This will be done at an appointment with your loved one to ensure only a person with an established relationship receives proxy access. If proxy access is required before your loved one has an appointment with a health care provider, you may complete a form and fax or mail it to the provider's office.
