Some locations within our network may be considered in-network with your insurance, while others may not. You may receive different bills from physician(s) involved in your care (e.g. anesthesiologist, emergency room physician, radiologist, pathologist, consulting physicians, etc.). If you have insurance, your estimate is created based on your benefit plan that determines the amount you owe, including deductibles, co-pay, co-insurance, and out-of-pocket maximums. The estimated financial responsibility may be due prior to time of service at some locations. This estimate may require you to enter your remaining deductible and co-insurance as well as any co-pays you may be responsible for.
