Coronavirus (COVID-19)

  • Video Screenings and E-Visits - Log in to MyLVHN to access these screenings. (Create a MyLVHN account right now if you don’t have one.)

  • COVID-19 vaccine - Everyone with a MyLVHN account will automatically be preregistered for a COVID-19 vaccine. If you are a healthcare worker, first responder (firefighter, police, National Guard or protective services personnel) fill out the essential worker questionnaire, which can be found by clicking on the menu button after you log into MyLVHN. You'll receive a notification when you are eligible for the COVID-19 vaccine and can schedule your vaccine appointment.

    All essential workers are required to show their employee ID at their appointment to receive the vaccine. If your ID does not include a photo, you’ll be required to show a government-issued photo ID as well. If your organization does not issue ID cards, a letter printed on corporate letterhead outlining your position along with a government-issued photo ID will suffice.
